Managers should fill out this form to officially report the score of a game.

Simply enter the data in the spaces below and click on the submit button. The information will be automatically sent to the TBO Scheduler and your division manager. If you want copy of the form, you may either use the PRINT button of your browser to print a copy of the form or enter your email address in the "Your Email" field.
